I know what you are talking about. I took it from HDFC Bank and had to provide them the format on my own.
They were kind enough to use the same language and details I wanted and as a result of that It was accepted fine by CIO.
I have a scanned copy of that and i can provide you the format, But maybe you will have to wait till tomorrow as I don't have the letter with me at the moment.

Do it both, try to write it to CIO now and after receiving PER to your VO. It is not a big problem. Don't worry.OMCANANDA said:Hi All,
