I am applying for the notes, have couple of questions!!

1. Can I get the notes for my spouse in the same request? If yes, do I just have to upload the consent form or I chose some options as well?

2. What type of information would you like to request? - What should I pick - Electronic File or Physical File.

I have a spouse too. When I applied, I just did mine and I can see my GCMS notes whats started for him and what has not... So I dont think you need to apply for your spouse unless your spouse is the principal applicant!

Also, select Electronic.... physical files will be scanned copies of all teh forms and documents you send! Electronic will be a pdf file with all the details! Thats how I did it!

Nope just ordered one for myself and it states the details for the dependents!
