Hello I am from Tamil Nadu (south part of India). I am presently working in Calgary and I am applying PR through Alberta Provincial Nominee Program. I successfully received my nomination in February and applied to CIC immediately. But my application package got returned back since I didn't include my birth certificate and I got the below message:

If you are unable to provide a birth certificate or secondary school records that indicate your date of birth and parentage, please provide an affidavit that has the following information:
1) Name of Client
2) Date of Birth of Client
3) Parents name (Mother, Father or both is acceptable)


Now my problem is I have a birth certificate without my name and also my father's name misspelled in it. So my father has now applied for name correction in the municipal office and I don't know how soon I am going to get the BC.

So I decided to look for school records and I managed to get my SSLC (10th Standard) school transfer certificate which has got my name, dob & father's name. But there is again an issue in that:

My passport contains only father's first name and it doesn't contain his initials or his last name. I think this is how most of the Tamil peoples passport would be like.

Father's name in my
Passport: XXXXXXXXXXXX
School TC: K.M.XXXXXXXXXXXX
Marriage certificate: K.M.XXXXXXXXXXXX

Whether CIC will accept if I resubmit my application just adding my School transfer certificate (or) do I have to prepare an affidavit highlighting this initial issue? More than 2 weeks have passed since my application got returned back, Seniors please provide your advise.

Submit an affidavit showing your date of birth, your name , place of birth and your parents name, exactly as shown in your passport. Your affidavit should be attested by Indian embassy or Consulate near to you.

thanks for your advise. but is it sufficient to indicate just my father's first name (as per passport) in the affidavit? because all of the affidavit formats I found on-line mentions both first name & last name.

Also what other documents can I attach along? I got 10th & 12th marksheet with my name & DOB.

Even i have same issue. I dnt have birth certificate. Instead of birth certificate can i submit my 10th marksheet, Income tax card, affidavit from parents and national card all these documents shows my DOB. please advise is this going to work.

Above answer to your query is 100% perfect. Just send them affidavit with details which they are asking.
 
Any lawyer can provide it to u in legal language. It should be notarized

I too have the birth certificate problem. Im live in Kerala, India. Is it mandatory that it has to be attested by Indian embassy or Consulate.

Correct me if I'm wrong. In India do we have Indian embassy or consulate?(I might sound stupid but I really dont know)
 
You dont have indian embassy or consulate in India. What kind of birth certif problem you are facing? I guess it takes about 3 -4 weeks to get the birth certif if you re-apply. Considering the fact that the NOC list and credential assessment agency is not announced, I advise you to apply for the birth certificate with your name on it, father and mother's name and other details (ALL IN ENGLISH) and get it done. If not for Canada, it will come in handy to apply for even Australia!

Surname: Kumaresan
Given name: Ranbir
Name of Father: Kumaresan
Name of Mother: Revathy

For this case, I have prepared a self affidavit for date of birth as below:

A F F I D A V I T


I, RANBIR KUMARESAN, resident of Suite#214, 1025 – 10th Avenue NE, Calgary, Canada do solemnly declare and affirm as follows:

1. I was born on July 03rd 1982 at Erode, Tamil Nadu, India.

2. My father’s complete name is KUMARESAN and my mother’s complete name is REVATHY.

3. Both my parents are citizens of India and they are residing at 123, RAM NAGAR, ERODE – 638001.

4. I am providing this affidavit in order to verify my birth details, as I am unable to attain birth certificate from the relevant issuing authority.

AND I make this solemn Declaration conscientiously believing it to be true, and knowing that it is of the same force and effect as if made under oath.

Hello friends, the details I had given in this post are not real. I just imagined and gave those details in order to have a better understanding.

The affidavit has to be given and signed by your parents on a Rs 100 non judicial stamp paper and notarized and should have the following details

1. Parents full name, age / DOB and residential address
2. Date and place of their marriage.
3. Date and place where you were born

You may also state that your birth was not registered with the local authorities.

My parents gave the affidavit for me as I did not have my birth certificate and it was accepted by CIC and I got my PR and moved to Edmonton last year.

what is RPRF amount that we have to pay at this time
I havt my family name in passport so what should i write in family name of Generic Form as i i left it blank this form failed to validate.
Should i write my given names as in the passport in family name box also
Is this creates huge problem for me plz reply
