4. I am providing this affidavit in order to verify my birth details, as I am unable to attain birth certificate from the relevant issuing authority.

AND I make this solemn Declaration conscientiously believing it to be true, and knowing that it is of the same force and effect as if made under oath.

Actually my application was returned for the same reason (missing Birth Certificate) and CIO, NS has provided the same suggestion as yours (attach an affidavit containing applicant's full name, DOB and parentage) if unable to produce Birth Certificate. Since i am also from India and we dont get birth certificate (unless we were born in late 1980s or after 1990), it would be really great to see how you managed to get your affidavit. I appreciate your response to the following:

1) What document did you use ? an affidavit with the suggested details ? or you got a birth certificate from India? or did you get an affidavit from Canada?
2) If you have got an affidavit what format did you use?
3) Did you re-submit your application already ? or still under putting everything together ?
4) I got my whole application pack returned ? is it the same for you ? or they just simply send a letter asking you to provide the missing document ?
5) If they returned the whole application, are your preparing the whole application again (from 'zero') or just using the same documents and sending them after attaching the missing document ?
6) CIO mentioned that they will return my fee ? did they do the same thing with you ? if yes, how much did they return ?

Your response to some or all questions, in part or full, is highly appreciated.
