Application Forms

Hello Members and PMM.

I just want to make sure I get some more questions answered about this whole form application process.

Where and how can I put in the request for the open work permit? I don't see a box in any forms that allows me to check off that (5002). Or do you just include the 490 dollars for the Right of Permanent Residence Fee ahead of time?

On form 5002, a13 and a14 are the residing addresses are where I am temporarily staying at? Correct?

Can I get Appendix A, B, C and 1017-scl taken care of at the country I currently reside at? If so, for Appendix A in obtaining the police certificate, where it says current address in Canada, then I should put my COUNTRY's address in the US or put down the Canada Address that I intend to stay at?

Appendix C for photos, would 4 be enough? 2 for the app and one for the medical form?

On 5002 Schedule 1, I can't remember my exact dates of one of my past employment (like from 1995 to a date I forgot when I left the company sometime in 1998 or 1997.) Would that get me in to processing delays?

Do I need to include the checklist (Form 5443) along with the application?

My city of birth is Hong Kong (but when I was born, it was british.... now is under China) so what is the COUNTRY I should put down as. Will they understand?

The checklist asks for joint account info for common law partners, but if we are applying as spousal sponsorship, would it help or would we NEED or not need the information on Joint account.

At the end of the questionnaire where it asks for additional information that would help prove that our commitments are genuine etc, what else would help prove that we are genuine? like deposits for upcoming banquets, receipts for purchasing celebration of engagement, civil ceremony? etc?

On the checklist #2 under the sponsored, it asks for proof of status in to Canada. Since I am a US Citizen and I have 6 months without the need for any permits etc. What would I use to show that I am entered validly and within a valid period of time? (specifically, it asks "indication of status in Canada (visitor visa, work permit, temporary resident permit etc)

0n form 5481, why is question number 4 defaulted to answer as "1" and if i am the only person she is sponsoring, then she would put "1" on #5 as well. Then in Number 7, she would put down "2" as the answer? I just want to be sure all the questions are answered correctly.

On the guide it says something the applicant have to right to purchase OH insurance? Can you briefly touch into that when you have a free moment? When can that happen?

Thank You PMM and Members.
Confused.

Information on the number of photos to supply varies by country and is in the Applicant Checklist found in each of the Region Specific Forms sections. Here’s a list:

Africa – 8
Asia, General – 9
Asia, Southeast – 5
Asia, Central – 8
Bermuda – 4
Caribbean – 8
China – 5
Europe, Eastern – 8
Europe, Western – 8
India – 6
Latin America – 8
Middle East – 8
Pacific – 5
Philippines - 9
Puerto Rico - 4
Saint Pierre & Miquelon – 4
United States – 4

Also, the DMP who conducts your medical exam will also require a number of these immigration specific photos – consult with the DMP before having photos taken so that you know how many to order in total.

Do I need to include the checklist (Form 5443) along with the application?

yes

it would help to prove your relationship is genuine and long lasting.

At the end of the questionnaire where it asks for additional information that would help prove that our commitments are genuine etc, what else would help prove that we are genuine? like deposits for upcoming banquets, receipts for purchasing celebration of engagement, civil ceremony? etc?

letters to each other, photos of you too doing life stuff or just hanging out, wedidng photos, cards, emails, phone records, anything with both your names on it,anything tying you two together before and after marriage.

Thank You PMM and Members.

ok thats my two cents worth, but note i am not an immigration expert and i would verify my answers before setting them in stone, just trying to help. I got the photo part from another forum i belong to so its not my original thoughts, but the person who post it is very knowlegable. Hope it helps a little.

Where and how can I put in the request for the open work permit?

You include an IMM 1249 with the application and the $150.00 fee.

I don't see a box in any forms that allows me to check off that (5002). Or do you just include the 490 dollars for the Right of Permanent Residence Fee ahead of time?

You can pay the RPRF upfront or when you attend the landing interview.

On form 5002, a13 and a14 are the residing addresses are where I am temporarily staying at? Correct?

Yes.

Can I get Appendix A, B, C and 1017-scl taken care of at the country I currently reside at?

Yes,

If so, for Appendix A in obtaining the police certificate, where it says current address in Canada, then I should put my COUNTRY's address in the US or put down the Canada Address that I intend to stay at?

Appendix C for photos, would 4 be enough? 2 for the app and one for the medical form?

Read the instructions.

On 5002 Schedule 1, I can't remember my exact dates of one of my past employment (like from 1995 to a date I forgot when I left the company sometime in 1998 or 1997.) Would that get me in to processing delays?

No

No

My city of birth is Hong Kong (but when I was born, it was british.... now is under China) so what is the COUNTRY I should put down as. Will they understand?

China

The checklist asks for joint account info for common law partners, but if we are applying as spousal sponsorship, would it help or would we NEED or not need the information on Joint account.

You need evidence to prove that you are couple not just a marriage certificate.

At the end of the questionnaire where it asks for additional information that would help prove that our commitments are genuine etc, what else would help prove that we are genuine? like deposits for upcoming banquets, receipts for purchasing celebration of engagement, civil ceremony? etc?

Statements from people that know you, letters, emails, telephone bills, etc.

State when you entered and explain you weren't issued a document.

0n form 5481, why is question number 4 defaulted to answer as "1" and if i am the only person she is sponsoring, then she would put "1" on #5 as well. Then in Number 7, she would put down "2" as the answer? I just want to be sure all the questions are answered correctly.

You can apply for OHIP once you get a letter from CIC (if you are in Ontario) that you have passed the medical.

Can you briefly touch into that when you have a free moment? When can that happen?

Thank You PMM and Members.
