For an individual person, there are some application forms that are not applicable. Since CIO requires that all documents should be filled out, where on the form should "Not applicable" be written or should these forms not be included?
You do NOT need to include the forms in your application package that DO NOT APPLY TO YOU. Just make sure to leave such forms unchecked in the document checklist.
