- Some applications have been rejected because the PCC was issued before receiving the ITA. Is that true ? In my case my PCC was issued in the 13th Nov, my ITA 4th Dec and my AOR is 8th if January.

I think the agent was reviewing something and marked it to review later. Then the agent reviewed and changed the status back to passed. I'm just guessing it's nothing wrong.
My meds will expire in July 2016, maybe they are checking to see if we have enough time to finish the process before medicals expire.
I don't want to do medicals again. Lots of money.

Did you submit appointment letter, Salary slips, salary bank account statement etc in support of employment letter.
Also, did you submit six month bank statement apart from bank letter.
I find your case quite strange and unique. I haven't noticed such requirements in the past especially the organization hierarchy chart and Insurance docs.

Compulsory military service card
POF most recent three bank statements (Which I wonder if it is recent three month or recent three bank accounts, which I only have one account)
Organization chart
Social Security Organization
and Companies staff insurance details
and Interestingly, the day after this request I received another mail, saying Background check in Progress.
Please advise me.

I think they are talking about three months and not three banks. To make your organisation chart more authentic you can get it signed by the same person you signed your employment letter. For insurance, you can share a copy of Insurance card.
BTW, which country are you from and what is your category- PNP or FSW, Inland or outland
