I have a question, for Background Check, is that mandatory that all applicants need a background check from CIC, or it is optional and randomly picked by CIC to check?

My ECAS is now 4 lines long; they added 2 lines about my meds being done and AOR being sent to me (duh...).
My BG check still hasn't started, even though it's been 18 days. Is it normal for them to take this long to initiate BG check? Does it mean they see a problem?

All applicants (especially adults) are usually checked for security and legal reasons.

At the top of the form they mentioned this same number as UCI.

I have submitted my application on 10th November and haven't seen any progress till now not even access to ECAS. When i fill out the info in ECAS form it shows whether u r putting info wrong or they have not started ur file. I have seen majority of guys got some response from them. Is everything fine with my file, I mean is anyone facing same issue?

Yes that is the uci and you shouldn't write your info here. I suggest delete it. Also you only have to submit the 4a form.
