In the tracker, I found the applicants who have changed status are FSW or PNP. For CEC, almost no one has medical passed. Why?

Not just for Jan applicants. This is the trend for November applicants as well. However, CEC applicants are getting PPR much faster than the rest (One applicant received PPR just 6 days after medicals passed) but it took around 65 days for medicals to pass.

most have not got medical passed, as per this forum and tracker.

So, say, if the meds status changed to "Passed" or any such changes take place in my account, will I receive an email for them? Or do I have to go in and check (as I do everyday) ?

yes, you should receive an email about your application being updated, however, this email sometimes takes up to 24 hours to arrive.
