IMM5406 - is for additional family information form. (This form is to be submitted by the principal applicant and the spouse separately)
IRCC is asking your for family information, not additional family information. So you can use IMM5645SE and declare your family. IRCC will ask for this from is they want to ensure that your immediate family is present in Canada or not or check their credentials.

You do not need work permit anymore after that (they will tear your WP once they validate your COPR anyway) and can legally work without any problems. You only have to wait 4-6 weeks to get your PR card which will allow you to freely travel outside Canada again.
You have to report your new SIN to your HR compensation team and CRA as well. I would suggest to get your taxes done for 2016 before you validate COPR just to save the hassle of waiting for SIN to get updated before you can file your taxes.
