I filled them in using a computer and used Capital Letters all through, but I guess you can use the format that you suggested as well.shibleebd said:HI BobbyB

That should be fine in your case. In my case, for example, I have a few clients who have gone into Administration "(Burst)", just Last Month. We have worked for the same company for more than 10 years since graduating from Graduate School, which makes things really easy when gathering documents. Luckily for us, we have now everything except for police certificates.
