My advise to you dear , is that you call them up by telephone and explain to them what happened maybe your case is different than mine and they need you to fill a different form . Good luck.
Call warsaw visa office they are nice and helpful , ask to speak with someone from the immigration section
They will hold the line and check your file on the spot and answer your questions.

I could not find it on the site , however after thinking , you should call them , they must have forgotten to send it in the e mail .
Also you cannot print it from the site cause it should be printed with the e mail they sent you .

And yes, you must land before you meds expire and in all cases, you must land before the date that will printed on your visa once it comes to you soon inshallah...

Our visas will expire mid June so we are planning to land around the first week of june as the kids will be at school until then. So we will land only few days (2-5 days) before visa expire!!!
We will only stay for 2 months, consider it as a vacation, and come back to Amman for another year.
We plan to move Summer of 2016.
