Actually in Pakistan anyone who is authorized to attest the document can attest such as Public Notary Commission,A govt employee ,a lecturer , a doctor etc.
As far as the address is concerned I am sorry I dont have any knowledge about it as I send my application in 2007 directly to London whereas you are applying under new rules in which you have to send your application first to CIO ,Sydeney,Nova Scotia so its better if you refer to the posts of anyone who has applied recently or visit the STICKY thread of some of master posts of qorax
Moreover I will suggest you to visit official site www.cic.gc.ca where all the details are mentioned .But still if you feel difficulty in anything ,come back to this forum and we are always here to help you.

Sister I am pasting an extract from the Information Guide which I used at the time of application I think it will be valid till now.

Certified copies or notarized documents
To have a photocopy of a document certified, an authorized person must compare the original document to the photocopy and must print the following on the photocopy:

‘‘I certify that this is a true copy of the original document’’

the name of the original document

the date of the certification

his or her name

his or her official position or title

his or her signature
20 Federal Skilled Worker Class
Who can certify
copies?
People authorized to certify copies include the following:
In Canada:

a commissioner of oaths

a notary public

a justice of the peace
Outside Canada:

a judge

a magistrate

a notary public

an officer of a court of justice

a commissioner authorized to administer oaths in the country in which the person is living

Please keep in mind one thing. Every applications is unique. So many guys who applied in 2009 and 2010 whose files were transferred to CHC london like yours have received medicals. It means they are working on the files. And secondly you can withdraw your application anytime and take your money back :)

Bro. I think this word is looking like hypothetical/predicted "many Pakistani applicants" and it should be "Pakistani applicants". Secondly

need to change this para "many of us have sacrificed other good career options just in favour of Canada as we think your great country Canada is the best place in the world to go and settle down. Our family lives have been affected badly and many of us are now suffering from depression and anxiety but this long wait is not coming to end. " Because they clearly mention that do not leave your job and please not make any major changes in your routin schedules before you get a VISA.

yar ismael this is a formal letter. we are focussing to convince that how important canada is for us and how far we have gone to get to canada. the word depression and anxiety is to make them aware that this is causing so much problem to us. while it may not be true in reality but thats how formal letter is written and the system works.

do you get my point??

finally yes this letter is open for changes. this is not final yet. let others to comment as well then we finalise it

This letter is just a reference every one will send his own if you sense some lines doesn't fits you than u can make amendments. i appreciate pittabread for his effort and time
as far as i know not a single applicant whi sent their dox to london in april did not get medical call which is unfair.
