I have never been in any other country for more than a month, but do have a lot of travel. Do I inform them every time I go out?

after a long time! where have u been?
no need to worry. there is no such form for us when we applied (post june 2010 applicants: SGVO)
however, this from was needed by other VO when we applied but not required by SGVO.
From 1st junly 2011, this form is mandatory for all applicants.

not at all. However, if you stay 6 months abroad then you need to get PCC from that country and submit it to SGVO
 
Just to notify one thing, please correct if I am wrong. I did not see anywhere in the forms where they asked for secondary certificates (pls check if any changes in the new forms). As per the old forms, all they asked for are post-secondary documents. That means University degrees or vocational certificates and transcripts. You can only get into universities when you finish 10 years of secondary education. So, if you don't provide SSC and HSC answer scripts, it is perfectly all right. In my case I did so and already have MR. However, giving additional docs will not hamper.

agreed..got my visa without submitting my SSC certificate/marksheet.
However, i think it's wise for all new(july 2011) applicants to submit SSC certificate+ marksheet

Those were business visit and 5 days stay maximum with business visa.
