Bear in mind that you will be ordering them on behalf of your wife since she is the one being sponsored.

So first form "REQUESTER INFORMATION" is all YOUR (sponsor) info.
*Are you requesting information on your own behalf? Pick NO and then choose the E-mail option as opposed to CD


2nd form: "ACT AND RECORD SELECTION"

Please select the Act under which you are submitting the request
ACCESS TO INFORMATION ACT

Which type of records do you wish to request?
CASE FILES

The rest will all be your wife's information (Principal Applicant)

File Type: Immigration File- Permanent Resident

Last form: "ATTACH DOCUMENTS"
This is where you attached the signed consent form found here: http://www.cic.gc.ca/english/department/atip/form-imm5744.asp
To fill this:

1) Your full name: This is your wife/principal applicant's info
2) Spouse info: This is your/sponsor's info
3) Designated individual: This is your/sponsor's info
4) Declaration Check both YES boxes
5) First box is your wife's signature
Second box is your signature
Date
You can ask her to sign the form and scan and e-mail it back to you. Afterwards, you can sign and scan and attach the form along with the request.

1 - afghanistan's cases are transferred to New Delhi, India now a days and the average time for 80% of cases are 15 months. But it varies for every person it could less or more than that time.
2 - Yes India is faster seeing the trend
3 - it depends on the office and application. some get it request and some doesn't. It's your luck.

Has anyone else been asked to complete Schedule A again, recently? We received an e-mail to complete Schedule A and also proof of Medicals ??? This is just another thing to drag the case in to the new year...inshAllah patience will pay off!

Yes my husband got it too just 3 days after he was asked for remadical ! On sept 8 ! It's required for the background checks , meaning right after they recive it they will start the check ! And make sure to match it just like the first one so you don't have 2 different answer in the new one !

Oh okay, that's what I'm thinking too....but my hubby did his meds in June, so his background checks should be done by now, right? Do you know if your husband sent it in by mail? or e-mail? Did he send it to this e-mail: DELHIIMMIGRATION @ international.gc.ca Thanks Sadaf

No if they have asked for this now it means they haven't started the background check yet ! It will indicate this in your recent notes too ! My husband send it to me since my cousin was just coming from Afghanistan I took it to my MP she send a scan copy by email and I also mail it to them they asked for original copy's of the form so make sure u send it by mail too ! Send a soft copy in email as well so it reaches them on time

Inshallah it will ! Security check and background checks are 2 different thing! Inshallah it will be done soon don't worry, and you are most welcome ! MP usually have an internal email contact! The one that you said above is correct dear send it to that one !

I'm not sure if everyone receives this or not...some people get it some don't. My husband also did his meds in June and they sent it to us just recently. Schedule A is one of the forms that you needed to fill out with the original application, for us they said to complete for 2012-present; so what jobs he has had, any further education etc.

Thanks hun, I think this is the last stage. One of my friend she got it too, and her lawyer told her it will be the last thing which they doing in cases... Hope you get PPR after this. I hope this week all the pending cases get good news inshallah. Khoda jan miraban ast.

That is so nice to hear! Do you know if her husband's background checks were completed when they asked for the form? As per my MP inquiry, I was told background checks were in progress in September, but Sadaf's husband background check started after they requested schedule A. I don't know what to think anymore
