Your name, spouse's name(if applicable), your father's name and mother's name and their relevant data. The same form will also be filled by your spouse and in this time your spouse will be main applicant and your data will be in spouse. If you haven't spouse write N/A in the particular column and do your signature in section A, otherwise write N/A in signature column.

but i am still not sure about when filling additional family for the spouse

in the main applicant part name will apeare or my spouse name , because i am the main applicant , but the form are for my apouse additional family information any body help me out plz

You should have one form as main applicant and another form for your spouse as the main applicant. dont confuse 'Principal' applicant with 'main applicant'. The 'applicant' or 'main applicant' is the person who is filling that form may be either 'principal applicant' or 'spouse'.

First get two forms of "Additional Family Information" now one form will be filled by yourslef in which you will be principal applicant and data of your spouse will be mentioned in next row "Spouse or Common law partner" column and then second form will be filled by your spouse now she/he will be principal applicant and your data will be mentioned in "Spouse or Common law partner" column.

I hope it clears.
