I am filling 5669E form for my spouse and I need additonal space for the form. In the additional form, should I write my spouse's name or what should I do? should I include my name(princial applicant) in the additional form. I am confused.

You can create a similar look and feel sheet in Word or Excel and attach it at the end of the specific form.

Put the name of the person that should have filled out the form. If it's your spouse who has to fill it out, then put his/her name on it and attach this sheet right after the form.

This Form is for Spouse of the Principal Applicant
Name of Spouse: yyyyy nnn
