On the additional family information it is under my understanding that both me and my spouse need to complete our own. Anyways, at the bottom of the form it asks you to list your siblings. Now it only provides 4 spaces and I have 6 siblings. Would I use 2 forms? or would it be best to write my other 2 siblings on an additional plain sheet of paper? Or because I could not fit them all on the form should I just write all my siblings on a separate sheet and refer the officers to it? Also do we need to put middle names on the application?

You do not need to complete that form. Only the PR applicant completes that form for his additional family members.

You should include their middle names, but I don't know if it's mandatory.

If you're asking if your husband should use his middle name in his application, he should show his name as it appears on his passport or other national identity document.

It says on the form:

The principal applicant, their spouse or common law partner and all dependant children must complete their own copy of this form.

Does that mean I have to do it as well?

Hi countrygal,

Your spouse does not need to complete this form. Only you need to complete this form. Your spouse is a Canadian citizen/PR so therefore does not need to apply for PR.

For your 6 siblings I recommend to put the oldest 4 in the 4 spaces provided, and then on a separate piece of paper write the following:

Additional Family Information Form (IMM 5406)

Applicant: Your Full Name
Sponsor: Your Sponsor's Full Name:
UCI: Enter your UCI if you know it

Section C: I have 6 siblings in total, 4 are listed on the IMM5406 and here are the other 2.

Sibling name / relationship / date of birth / place of birth / marital status / address
Sibling name / relationship / date of birth / place of birth / marital status / address

Yes enter the full names.

Note that the guide says to

If you need more space for any section, print out an additional page containing the appropriate section, complete it and submit it along with your application.
However I think it will be confusing having 2 copies of that form, one with all the sections filled in and another with just the siblings filled in. In my own application I did it the way I described (I have 5 siblings so I needed an extra sheet) and my application is just about there!

Haha okay, countrygal only the applicant needs to complete the form, so as you are the sponsor you should give the pen/keyboard to your spouse and tell him to fill in that form and the additional sheet.
