As form IMM5406 must be filled by applicant, spouse etc seperately, is it ok for applicant to leave fields for spouse blank (unfilled)?

Umm, why would you want to leave them empty? Don't you think if the information was not necessary it wouldn't be there in the first place? Note that your spouse would be the applicant and you will be the spouse in the form that your spouse fills.
