I just got an email from CIC requesting IMM5475 (authority to release personal information to a designated individual).. anyone here who got also the same request like me?anyone knows what kind of form is this?

It is like you giving authority to a representative of CIC to know the status of your profile.
Like if you are doing through consultancy/agent, If you give this form only they will be notified with your status update of EE.

Hi Sreeni,
How do i get to know the name and location of the office?
Also, i had removed my spouse at the designated individual during ITA submission to avoid any additional paperwork. Should i keep the same stand or add her back?
