Addition Fmily information sign by spouse

Hi..

Regarding Additional Family information(IMM 5406), it is specified in the checklist that this form should be completed, dated and signed by
*You as the principal applicant
*Your spouse or common-law partner
*Each dependent child over the age of 18 years.

But, my doubt is where should my spouse fill and sign in the form as there is no additional boxes provided (only provided for principal applicant).

or should we take another separate copy for spouse.?

If we should take another copy, should it be completed with his family details..? his spouse (relation: me), his mother, his father etc...AND LEVE APPLICANT COLUMN blank???????????????

Please clear my doubt ASAP. its very urgent..

Seperate form for spouse and signed by him/her. His parents, bros,sister info to be added.

Applicant mean Primary applicant same for both in spouse form and PA form

simply fill two add family information one by you and one by your spouse and in your spouse form she will be applicant and in spouse column your name will be......your children information will be the same in both forms........in your form your family information and in spouse form your spouse family information simple...........

I know why are you confusing ........ok now fill up your spouse form and write your spouse mean husband name in principal applicant block and in second colum where spouse/common law partner there write your name ...........once i was too confuse like this..........

Now I got it but coolguy mentioned other way, that is applicant in anywhere is a principle applicant ?
Because my husband is accompanying me, he is not a principle applicant...

do not worry i was confuse like you that how i can write my spouse as principal applicant in additional family information lolz..........but later on i got it that information is about your spouse so only in this column in the whole application pack only once your spouse is written as Principal applicant cause he is disclosing his or her personnel information and remember every application form must be properly signed i sent you text if you need any kind of help feel free to contact ..........

Dear Sunny,

I would agreed with awai ali. Its quite logical. When your husband is filling the form will he write your mothers's name as his biological mother? NO. So on your husband's form his name should be as applicant and your name as spouse and HIS signatures come on form. In your form your name comes as applicant and your husband's name as spouse and YOUR signatures come on form.

REMEMBER THE FORM SAYS APPLICANT AND NOT PRINCIPAL APPLICANT. I hope its clear now.

P.S. Do not field any field blank. Remember to write N/A in field not applicable to you.

Choose UPS for courier that is awesome service for delivering application on time ........never ever use DHL....they charge more and delayed enough at least in Pakistan .......

Anyways now what next Ali how do we know the status of the application ? got to know that we have to wait for 2 months ?

ok do not worry ....now they will intimate you through email that is called Pre Eligibility Review (PER) there will be UCI (Unique Client Identification Number) through that you can check your application status on www.cic.gc.ca and in which category do you apply ???? mean what NOC????
