My question is: From dropdown(Type of application/enquiry) what category do I choose?

Hi I got the same request in the email today, I sent it via fax, my visa officer is ZKA, did they respond to you yet if they approved you as sponsor already? The application packge wz received May 4th, so mine actually took longer for them to open and review than most of the people here..are you sure they ask for NOA not Option C print? note that they are two different documents. Who is the officer that signed the letter? Is it Officer ZKA?
