1. Biographical page of passport(s)/travel document(s)
Does this include the copies of the visas previously issued for Canada or other countries or is it just the page with your photo and other personal info?

2. Fee Receipt
I am making a family application. Under client id & date of birth, should I include the info for all applicants of just the person that is paying the fees on behalf of the whole family?

Please advise.

1. Copies of the page(s) with your photo/personal information not the pages of the visas.
2. You need to indicate the client ID numbers for all applicants as they need to know who the fees are being paid for. If your in dought I suggest you go to a YMCA who has experience in these things and they will guide you accordingly. If you call CIC those reps can/will tell you crap

I wrote my client ID and DOB but included a copy with each of my children's application. In the covering letter I mentioned the names of all applicants pertaining to whom the fees were paid.

Just as indicated above yesterday - you need to include all client ID for applicants as they need to know who these fees are being paid for

You can just write your own client ID as you are the one paying for it. This will be a reference as to who to give in case of a refund. You only need 1 copy for all (family applicants) as you are sending it in one envelope and will be processed together. There is a breakdown as to how many are being paid for(adults&children) in the receipt. You don't need a cover letter for it. Good Luck
