I don't think you have anything to worry about, although it seems your file maybe got lost for a while and has now been found. Since you applied more than 15 months ago you are entitled to call CIC and ask them about the status of your file. I would suggest doing that, but I think you're on track still (just a little late).

Full time means at least 30 hours. Usually 40 and up to 48 a week. After 48 hours a week it's over time and employer tend to avoid that number. You should be fine as long as you don't go below 30 hours a week.

1. My wife previously refused two times open work permit from back home, is Case officer concern about her refusal
2. My wife refusal can effect our PR application
3. From the notes what is case officer impression about our file
4. Which stage my file is now, begging or middle or last
5. On the top the notes said Level: Eligibility what does it mean

Try to find your visa office. Because some files are in Ottawa and some are in Sydney. Only visa offices can give you precise answer. Email them. They will let you know if they want any document or not? Or you have to wait for further processing without any document. Phone inquiry sucks. Email them for your accurate answers.
 
Received request from CIC for medical re-assessment within 30 days. Somehow its a good sign... our application is now moving.
