    It was just a form attached to the document request letter they sent me, like a simple excel worksheet where you are asked to fill out a couple columns. According to GCMS timeline, it was sent out right after eligibility passed. I'm really curious what the next steps are...

    I'm FSW inland AOR March 16 2017 Med pass May 12 2017 BG IP2 since late May GCMS notes show eligibility, medical and criminality all passed in late May, but security is not started. They asked me to submit a gov service details form in late May. Were you asked for the same doc?
