I have read somewhere in this forum itself, that we need an affidavit for name change document. In the CIC website there are two separate placeholders to upload the Marriage Certificate and the Name change document

If you are uploading any affadavit, then I guess they have to be notarized. I was asked to upload a legal name change document, due to change in my name after marriage. For that I had to upload a notarized affadavit.

Yes, you only need the soft copy of the result to create the EE profile(There is a reference number which you need to enter), but you will need to upload the scanned version of the physical copy after getting the ITA

I did my ECA recently from WES. I received the hard copy approximately 1-2 weeks after the result was declared online. Did you try to contact WES about the status of your result?

What explanation can we give in the LOE? Should we simply write that we were unable to get the required documents, hence have removed the Work History?

You have 60 days to consider that. You can either Accept or Decline the ITA before these 60 days. If you do not take any action before that, your profile will be disqualified, and you will have to create a new Express Entry profile to get back into the pool.
