As I know, CIC send thousands FN emails on 4/14-4/15, only 2 days. Package received by CIC from the end of Nov. to Jan. 22, all these guys got FN at this 2 days.

yes, they will return your application package after 3 times charge fail. it is a fundamental rule for CIC.
