Birth certificate is another document that CIC requires. You have to obtain it. If you cannot obtain BC, you can produce an affidavit which must also be notarized.

Can someone list which are the forms I want to include in my application under FSW 2014? I am the principal applicant and wife accompanies, no children. I will be forwarding my application through an agency engaged in such immigration services.

My suggestion is that, it is better your spouse takes IELTS if she can obtain CLB4. So that you will be in the safe side even if you loose some points somehow in CIC and you can compensate with that of your spouse.

An affidavit declaring your date of birth would be enough which must be signed and stamped by a Notary Advocate. This affidavit must be declared by your father if you are the applicant.
