Mailed My completed RQ today. Have not received my health history from OHIP though, will mail it later separately. wish me good luck :-) I will post here when I hear anything from CIC.

My understanding after reading the RQ form is that we have to provide information on the period since the first arrival (landing or date you entered Canada to live, whatever is earlier). But supporting documentations must cover only that 4-y period.

Will you meet your due date if you wait for CBSA report? I read that some people sent their documents without this report with explanations that CBSA report would be mailed separately upon receiving.

Andrew2012, I think you need to translate only those stamps that are not in English or French. Can anyone advise me do I need to send original bank statements and Notices of Assessment? Or photocopies? Thanks.

I did not find this requirement. Am I missing something? On the last page it says - Health record detailing the dates at which medical service were received - Immunization records for children Is this Health record should be OHIP record? Would not simple record from a family doctor be enough...
