you should upload all documents related to previous traveling, which includes old passports. I uploaded the new and old passport stamped pages as one pdf file on the passport field.

Well mine didn't have that field, which I considered weird. But I thought at that time that they have received a copy with the CAQ sent to them, so that's why it wasn't required, and I was wrong. From experience submitting visa applications, this is the worst system I've encountered till now.
