You have to take IELTS exams first. Based upon that result ,your academics and workex( If any), you have to apply to any college/university for study. If they select you, then you will be provided offer letter( Acceptance Letter). And then you can apply for study permit ( Student Visa).

You can fill the IRCC webform for case specific enquiry Also ,you can contact to the VAC via email to quote that your application has passed the normal processimg times etc If they reply, its good for you. Otherwise you have to wait. Ask for some extension to your school.

Medical reports are valid for 12 months only. And medicals are required only if you want to extend your study permit for more than 6 months. Otherwise it is not required if you want to extend it upto 6 months.

Yes you can apply it with application of study permit for your wife But you will only get OWP after your wife's study permit has been accepted for timings, it is nearly 9-10 weeks for OWP and 5-6 weeks for Study Permit now. But it is changig. U can chek the latest here...

Not at all. Medical reports are valid for 12 months. It may be because VO has seen something suspicious about your health, and want further clarification. But at last, only they know. Better for you to not delay, and submit them as soon as possible.

Medical Reports done at Panep Physicians provided by CIC are valid for 12 Months dear. So 4 months is not a big deal.
