I think the 2 documents got added recently if I am not mistaken . If you have resided in Canada for 3 years - Then get the Ontario Name change certificate first and then use it to update your Canadian DL . Then you have two public Canadian documents .

I assume you are referring to your Indian Passport , I think you should coz if you change only Canadian documents eventually you will be applying for Canadian Citizenship along with that you would need OCI/indian VISA and have to give past indian doc references , I think to avoid any confusion...

Well chances of approval are dim , but if stronger ties are convinced to the officer and the duration is kept very short . It can work . In my experience i have seen it working .

Yes even i noticed it today - I used (06-2019) whereas now its (09-2019) and i have sent my application on 24th Sept and got received on 27th Sept . I believe only getting an AOR can answer the above question .I don't see any content difference though between two forms , Did you ?
