The error message might be because this is an old request and it is passed due! - another reason could be your internet browser, you might need to change your internet browser - I found Google Chrome to be the best!

It all depends if you are from a non exempt visa country or not - if you require a visa, the PPR will mention that passports have to be valid for at least 6 months and they must include at least 2 blank pages

PPR should indicate the 6 months validity and at least 2 blank pages if you are from a non exempt visa country

Have you got your fees refunded?? if yes - then you should expect a request to re-pay and when you will upload your payment receipt you will see if the same error happens to this fresh request or not, if it did go, they may update your file with a new fresh Schedule A request. In the meantime...

Schedule A is a normal and frequent request - but not sure why some got it and some didn't. All what I know it is always a good sign :) As for the visa refusal, you should always state the reasons for the refusal and there is a space right after that question to explain your reasons...

Don't freak out, this is normal. It happened to me, got an email notification that my application has been updated, when I checked myCIC, found everything went back to where things were before the cancellation (i.e. application status open, BG in progress, Med Passed......etc.) and it happened...
