In the main form IMM 0008, In "consent to release information" Section I mentioned my name as KAZI ISLAM but my full name is KAZI ZAHIRUL ISLAM. I did it as they mentioned to provide (first name, last name). Will it be a problem not mentioning the middle name here?

Thank you both! A trail question I forgot to ask: I made color copies of my certificates and transcripts and notarized them. Is it okay to notarize or I have to send them without notarizing? Also color copies are fine, right?

Thank you for your prompt reply! Actually I meant the weight of the paper not the size. 80 gsm is fine, right? Or should I use 100gsm?

Your feedback and sharing of experiences really helped. A small question: What kind of paper I should use to print out all the forms? I meant 80 gsm or 100 gsm as they said to use bond quality paper. What does that really mean? Any feedback is much appreciated!
