if you are applying to CEC, you can leave the UCI field blank; CIC has multiple ways to find it for you. however you need to correctly fill the date of your last entry to Canada if you lost your old passport.

pay attention that the biodata form is an html document. depending on your email client you may have issue to open it. I saw on another post yesterday that senior member of this forum sent that form to another guy with same issue like you.

you can put your residence Alberta. if you move out to another Province (Except Quebec)... you simply send an email to CPP-O to change your address once you get your AOR and file number

if you live in Quebec with an address in Quebec at the moment you apply to CEC, you are not elibigble and you need to go through CSQ.
