Should I submit the reference letter from the latest branch or for every branches I had worked for?

Could anyone please upload a format for reference letter which had been submitted to CIO?

Did you get any medical request or further documents to submit when your explanation was showing "your application was reviewed and started ..........." and what about your NOC? Visa Offices are doing lots of peculiar activities.
