Once you have submitted your application, you don't need to report vacations to CIC. And yes you only need a new PCC if you have to apply again later after your vacation. You will scan your fingers on an electronic device (similar to what the embassies use), then the print it out on the card with their printer. Their printer prints directly on the FBI card, they usually give you 2 cards for the $10.

I am still yet to submit, but yes, I will include it in my documents to be submitted. I wrote a LOE too to explain it, mentioned the documents that has which names and the ones that has the other names and said its one and the same person, included the sworn affidavit. I think it should be fine.
