As far as I understood, you need to complete the form again and fill the required section in the application this time. Along with that you need to attach site authorization form. That's what we did, we are still waiting for response

Hi, my PHAC approval got rejected because the place for the quarantine is same as the location where care the patient is required. They've asked us to mentioned that on form and include site authorization form. Where on the form you should mention that both locations will be same??

It's really strange that when we submit our profiles we are confident about the submitted documents but as the time progresses, you start to have doubts and start fearing the worst.

That's not true. My AOR is 468 and I know people who barely made the cut-off have already recieved their PPR
