I called the CBSA Ottawa office right after I received the report. The person looked for the missing records while on the phone but could not give me a clue. Based on the circumstances, I wonder how CIC can rely on such faulty and incomplete report to determine residency of an individual?

I agree. I recommend writing a detail cover letter stating documents that prove a specific area.

You have to cover 4 years prior to your signing date for both questions.
