    Dear Documentary proof is enough no need to send pics. It is better to get Marriage Certificate from Union Council as well which will be in English & Urdu. Further your wife should have valid passport as well.

    You need to get police certificate only if you stayed more than 6 months...... Termination of job due to bad performance is not a crime hence it will not effect on police record even if you stay more than 6 months.

    IELTS test should be valid the date it arrived at CIO you will be fine with that. For question 2 important is job description rather than title hence you will be fine.
