if you have already filed for divorce,you should apply as separated. You will not provide any documentation foe the spouse except court proceedings until judgement. you can apply.

forst the first number is your UCI number. You are only submitting the IMM5475 . However since you didn't authorise anyone else to view your file ,you should not submit his file instead send a LOE stating you have not approved any person yo view your file

when you click legally separated,it doesn't show you the option to input spouse details. I filled legally separated and didn't get questions about spouse details.
