What date did you submit and when did you receive the letter? Other applicants just wrote n/a on the checklist for the Use of a Representative but if you think including a note will make things clearer then maybe you could do that. Atleast you can explain to them that you don't need it anyway. :)

May I ask what you submitted as proof of your relationship? VOs usually look for evidences showing that the relationship is ongoing so the non submission of the convos may raise a red flag. Some VOs are kind though and may give you a chance to submit additional proof if they find the ones you...

I think just for consistency she can just use her maiden name for the NBI clearance. I used my maiden name when I got my NBI clearance and that's what I submitted.
