I received two letters about my file being sent to the LVO. The first one was in june, and it was from CPC Mississauga, and the second one was in September from CPC Sydney. Anyone else experience something similar? I’m confused as to when exactly my file was transferred.

It doesn’t have to be capitalized. When typing, legibility is what matters, so you can do it in either lower case or upper case. As for signature, just type your name into the box. They don’t expect it to be the same as your actual signature.
