    I think the standard advise in such a situation is to try and get employer's refusal in writing. This way you have proof you tried you best to get the reference in the required format.

    Why would you want to do that? It is a medical test not a morality exam. It makes you pee a lot, thereby 'flushing' your system. NOTE: the above is my poor attempt at wordplay, please don't take it as medical advise. Alcohol cannot be removed from the system in this way.

    I have to join a course and if I am unable to fly on 26th I wont be able to join. I had already provided the details of the course in the visa application. Would this be a good enough reason for them to expedite my application? Or does 'critical emergency' only cover health grounds?

    If your employment reference letter mentions your salary then there is no reason to provide any additional proof or LOE.
