Thanks for reminding me. First, i was very confused to put it in personal history or not. In my country it is abit special.. We can take paid paternity leave for one year. Then up to 3 years for child care leave without paid. It's a law.

I sent separately. Basically, I sent what i had in my hand. My dimploma first, then my master course transcript+ the requesst form. And the last one was my bachelor 's degree transcript. The university never sends document for any students, at least in my country.

It mays vary by country, but this is what i did. I got the small sealed envelop(Academic record request form + transcript inside) from uni. and put it in a big envelop (which has my uni. address printed on it). Then i sent it.

Congratz. Wha i can say is that my university staffs were really unhelpful. They intended to make me wait about 3 weeks to get the form. After 30mn talk on phone , it is reduced to 2weeks.
