That's medical form. I think you are getting confuse because of word Biodata in it. You need to print it and take it with you to doctor. "MEDICAL REPORT CLIENT BIODATA AND SUMMARY" You will receive Biodata and RPRF request separately. Thanks for replying...Yes i was confused about that word...
