I am not 100% sure, but there is a an optional document , called schedule1 In that form, they ask for travel history again and we can add as much rows as we want. Do check it.

You should not post acknowledgment from ITR. You need to supply intimation u/s 143(1) from ITR. I guess that is much better proof. You can request for duplicate intimations from incometax website.

Gcms notes contain detailed notes why your visa got rejected. The reason we get on refusal mail are usualy generic. There are two ways to apply. One is...if you know someone from Canada, they can request gcms notes on your behalf. Fee is 5 cad. Other way is, there are website like getgcms...
