As per my understanding, PPRs are usually sent via emails however there are instances where it gets updated on CIC portal before being emailed.. I'd suggest you to inform your consultant about it or raise a CSE and update your email address..
Usually London VO are the quickest in issuing PPRs however there are many on this forum who are sailing in the same boat! Order GCMS notes if you need an insight on the current status or sit tight for another month or so :)
Yes for submission the passport holder must be present or an authorisation letter must be provided by the passport holder mentioning that you are submitting on their behalf. It is always better to visit VFS with all passport holders.
At this stage an officer will be assigned to review your entire application and once the officer is satisfied (most cases) you will receive your PPR . In the past BG in progress to PPR for Outland apps is anywhere between 1 day to 40 days.

Yes there has been many instances where files are processed in ottawa.. until PPR.. due to workload distribution to VOs that CIC has adopted, your file can be processed anywhere in the world.

Are you inland or Outland?
Most cases agents' reviews are readily approved by officers however there have been cases in the past where officers have rejected apps due to inconsistent NOCs/dodgy PoF etc. every case is different and will be treated at officers discretion
This will give you an insight on further review and PPR notes - mostly the officers are satisfied with agent review once BG has passed.

Yes I have observed the same pattern in some GCMS notes of last year which was received one month after PPR. The officer basically reviews the entire application before batching them up for PPRs - at this stage an application can be refused as well.
Looks like NDVO is committed to process apps within 6 months from spreadsheet it looks more than 90% is completed! 140days since AOR , fairly I'd give another 30 days :)
