Your PGWP will be rejected if you work more than 20 hours a week. Reaching to PR is a different scenario. I have seen people's with rejected PGWP due to the same issue.
You don't have to go through medical for study permit extension. As I just checked now, you have to select no in Have you gone through medical examination in last year and also select No in Have you stayed in Designated country for more than six months in last year.
