My bad, thought as they ask for addresses for the past 10 years, the same applies for police checks

We are now in December 2016. you need to provide police checks for all the places you have lived in for 6 months or more in the past 10 years. 2004-5 is more thann 10 years ago. Why would you need to provide the checks then?

Thanks the pilot, but I don't understand. It is because of position in the organisation that I was borrowed to carry out an additionnal duty, in my capacity as working in my nominated NOC. My reference letter will state that so I do not understand where lies the issue.
